Requirements for minor clients

 All minor clients will be required to have ALL of the following.

• State issued ID for minor.  

             (An ID for clients 10-15 can be obtained here)

• State issued ID for Parent/Legal Guardian.
• Birth Certificate for minor  

          (If the parent/guardian accompanying the minor is not listed on the birth certificate                                    guardianship papers must be provided as well.)

• Notarized Lucky Lady Tattoos Consent Form

         (Form must be notarized by an official notary BEFORE your scheduled appointment time.      The notary MUST use either a notary seal or stamp, we will not be able to accept a signature without a seal or stamp. 

*We are unable to accept any photo copies of documents*

• Age 16+
• Placement: We will not tattoo face, neck, hands, hips, butt, chest (female).
• Summer is the only artist that will tattoo minors, when booking make sure to 

              state that you are a minor.

• Tattoo designs and placement are done at artist discretion.

• We offer Ear Lobe piercings to clients ages 10-15 
• We offer most other piercings to clients ages 16-17 (We do not offer Daith, Septum, Nipple or Tongue piercings on minor clients)
